Please submit these forms in your next report. <br /> The information PHS/EHD requested in a letter dated July 11, 1991) <br /> (copy enclosed) has not been submitted to date. Please submit <br /> this information in your next report. <br /> It appears that the lateral and vertical extent of contamination <br /> has been defined. Pursuant to Article 11 (Corrective Action <br /> Requirements) amendment to Chapter 16 Underground Storage Tank <br /> Regulations (CCR) a "Corrective Action Plan" should be proposed. <br /> The corrective action plan should include a feasibility study to <br /> evaluate alternatives for remedying or mitigating the actual <br /> and/or potential adverse effects of the contamination. Three <br /> alternatives should be proposed, one of which can be quarterly <br /> monitoring. Discussion of the efficiency and cost-effectiveness <br /> of each alternative to restore or protect the groundwater beneath <br /> the site should be included. Documentation to support the claims <br /> and justify the assumptions put forth in the discussion is <br /> imperative and must be included. <br /> As stated in the July 11, 1991, letter PHS/EHD will conduct a 30 <br /> day public notice period on your "Corrective Action Plan/Final <br /> Remediation Plan". Please prepare an "Executive Summary" to <br /> accompany the "Corrective Action Plan". Submit two copies of the <br /> "Executive Summary" and "Corrective Action Plan" to PHS/EHD. One <br /> copy will be made available for public review at the PHS/EHD <br /> office during normal business hours. The second copy is for <br /> PHS/EHD staff review. Also provide a copy of the "Corrective <br /> Action Plan" and "Executive Summary" to the Tracy Public Library <br /> located at 20 E.
