I have included a copy of the"Cost Pre-Approval Request" form;please use this form in the future for requesting pre-approval of corrective action costs. For clarification purposes I have combined the total of the last three pre-approval requests into one complete request. With the following provisions,the total cost pre-approved as eligible for reimbursement for completing the June 1, 2001,Apex Envirotech Work Plan approved by the San Joaquin County Environmental Health Department in their June 7,2001 letter, is$138;444,00, This pre-approval corrects and amends the July 27, 2001 and the.
