Monitoring Equipment and procedures:
OVM with 10.0 eV lamp. Take background readings in the area you do not
suspect to be contaminated. Let instrument run continuously, or set on standby
mode. Take readings in the breathing zone upon initiating site work. For an up-
grade to be warranted, readings in the breathing zone must persist of time; i.e., 5
to 10 minutes. This is a judgment call. Spikes on the OVM above the action level
do not call for an up-grade.
ACTION LEVELS: If readings are 0 to <25 ppm above background in the
breathing zone, continue to level D.
Readings from>25 to 100 ppm above background, in the breathing zone, require
Level C (or leaving the site). Readings over 100 ppm in the breathing zone
require up-grade to Level B.
Team Members: Name Responsibility
Site Safety Officer Mike S. Sgourakis Site Safety Coordinator
Health and Safety Officer Lisa Nicodemus Corporate Health and Safety
Site Entry Procedures:
Locate emergency telephone numbers and route to hospital prior to starting any work.
Notify site manager of your presence at the site and your field plans. Prior to work on-
site, you must conduct a safety briefing with your sub-contractors. This shall include
informing them of the hazards associated with site work, and the chemicals anticipated.
For excavating - Position equipment and contractor upwind of the area to be excavated, .
and up-gradient of the drilling site.
Set up decontamination area upwind of the drilling site. This means get water and
cleaning solution in your buckets to wash off your boots and gloves prior to beginning
work. You want to set up the decontamination area at a sufficient distance from the
excavation or boring work to be reasonably sure you are not in an area that can become
contaminated by splashing water or flying dirt. Your support area is where you park the
truck, etc. It is the place you need to consider as clean. You do not want to make it
"dirty"by any of your activities. It is anticipated that work, will start in Level D,but be
prepared to up-grade or leave the site.
Inspect exclusion zone (the area where you are going to excavate, where you suspect
contamination, either "on-site" or off) for spark sources, paying careful attention to
electrical equipment. Do not start excavation or boring work until all possible sources of
