These services were <br /> connected with an environmental assessment requested by your agents, Souza Reality and Attorney <br /> Patrick Riddle. As you can see there has been no payment on this account since February 13, 1995. <br /> An agreement signed by your agent back in November, 1994 (also enclosed) stipulated that he would be <br /> responsible for fees for PHS-EHD services. Repeated calls to your agent have resulted in a referral to <br /> Attorney Patrick Riddle and still the outstanding balance has not been paid. <br /> Currently three groundwater monitoring wells are located on this property and they are not being properly <br /> monitored. San Joaquin County Well Standards require that all wells must serve their intended purpose, <br /> and a report of the activities is to be submitted to PHS-EHD on a regular basis. Wells not meeting this <br /> requirement are in violation of the San Joaquin County Ordinance Code and subject to formal actions. <br /> PHS-EHD recommends that you re-evaluate your site's current status and take steps to return it to an <br /> acceptable compliant standard. The monitoring wells are to be properly addressed and the outstanding <br /> account settled. Please have your agent remit the outstanding balance by August 30, 1996. <br /> Unsettled accounts will be referred to an agency responsible for collections. <br /> The monitoring wells should be monitored and/or sampled and the report sent to PHS-EHD by <br /> September 30, 1996. Monitoring wells not conforming to these minimum standards are in violation of the <br /> San Joaquin County Ordinance Code and will be required to be properly destroyed prior to September 30, <br /> 1996 under a permit issued by PHS-EHD. Failure to comply with the deadlines established in this notice <br /> will result in formal actions. <br /> Comment letters, such as this one, are also chargeable to your account as well as any formal action that <br /> may be required.
