The ERP <br /> is not updated.All n One Design (Chad Clark)stopped working for the business as a consultant in 2018 and he is <br /> still listed in the emergency response plan as an emergency contact. The listed phone number in the ERP for SJC <br /> EHD is(209)953-6200. This phone number is incorrect.The correct phone number is(209)468-3420. A business <br /> plan shall include the following emergency response procedures for a release or threatened release of hazardous <br /> materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> This is a repeat violation, Class Il. <br /> 8 HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program was not submitted to CERS Annually. The training Program has not been submitted to CERS <br /> since November 30, 2016.The training program uploaded to CERS is not updated. The Training programs for all <br /> employees and annual training, including refresher courses, shall include familiarity with the facility's Emergency <br /> response plans and procedures in the event of a release or threatened release of a hazardous material. Training <br /> shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> These training programs may take into consideration of each employee. Immediately prepare <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> On site to conduct a routine Hazardous Materials Business Plan inspection. There were seven violations during <br /> the inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br /> statement documenting the corrective actions that have been or will be taken for each violation, and any <br /> supporting paperwork, by January 08, 2020 . <br /> Note: <br /> Pat Coe retired as of December 01,2019 and is listed as the secondary emergency contact in CERS, and one <br /> the emergency contacts in the emergency response plan. Update this information by December 30, 2019. <br /> The facility is no longer storing propane, gasoline oil, and waste oil at this site.
