A typical well construction <br /> profile is shown on Figure 5 of the Sampling Plan. In addition, a check <br /> for the amount of $35.00 payable to the San Joaquin Local Health District <br /> for a drilling permit is enclosed. The requested workman's compensation <br /> form will follow under a separate cover letter. <br /> Please advise us when you plan to make an inspection, we will begin <br /> drilling operations on March 17, 1986. All inspectors wishing to enter <br /> the Field Avenue Site must check-in with the Site Safety Officer <br /> (Mr. Fred Wehrenberg) prior to entry and before leaving this site. We <br /> will require that inspectors comply with our Health and Safety Plan at a <br /> minimum. Your inspectors will be expected to provide their own Health <br /> and Safety gear and have some form of documentation which certifies their <br /> participation in an occupational medical monitoring program and that they <br /> have been trained to use the type of respiratory protection required at <br /> this site. If your inspectors cannot comply with this requirement, the <br /> Site Safety Officer will not allow their entry. <br /> In addition, we have contacted Underground Service Alert (USA) in <br /> Stockton so that all nearby underground facilities will be identified and <br /> marked. <br /> Sincerely, <br /> David W.
