Nelson, Chief Executive Officer, acting on CVIN's behalf and not <br />in his individual capacity (Respondent) (collectively, the "Parties") have entered into this <br />Consent Order and agree as follows: <br />1. Respondent own and operate CVIN-Stockton HUB, a communications <br />business at 4203 Coronado Ave, Stockton CA 95204 (the "Site"). <br />2. The CUPA inspected the Site on September 27, 2018. <br />3. The CUPA alleges the following violations of Chapter 6.95 of the California <br />Health and Safety Code ("HSC"): <br />3.1 The Respondent violated HSC § 25505(a)(2), failed to complete <br />and/or submit a site map with all required content. A site map was <br />not completed or submitted as part of the business plan. A site map <br />shall contain a nort <br />h orientation, loading areas, internal roads, <br />adjacent streets, storm and sewer drains, access and exit points, <br />emergency shutoffs, evacuation staging areas, hazardous material <br />handling and storage areas, and emergency response equipment. <br />-1- <br />CONSENT ORDER CVIN LLC <br />
