Please submit a permit application and the associated $89.00 fee as soon as funding <br /> allows. <br /> Please be advised that should the results of the proposed work, as outlined in the workplan, determine the <br /> full extent of the soil and groundwater contamination, the report to be submitted to PHS/EHD should be <br /> the Final Remedial Report. This report should describe the established extent of the soil and groundwater <br /> contamination plumes. It should also outline, in detail, a plan to remediate the identified contamination. t <br /> If the results of the proposed field work do not fully identify the extent of the soil and groundwater <br /> plumes, or if any remedial feasibility studies are needed to prepare the Final Remedial Plan, then a brief <br /> workplan for the work should be submitted and the work performed prior to the submittal of a report.
