' PUBLIC HEALTH SERVICES
SAN JOAQUIN COUNTY
JOGI KHANNA M.D., M.P.H. A
Health Officer
C;Glp p'i+P
P. O. Box 2009 • ( 1601 East Hazelton Avenue) Stockton, California 95201
(209) 468- 3400
October 29, 1990
Fred Beeson, Director of Engineering Crystal Cream & Butter Co. COPY
P.O. Box 1313
Sacramento CA 95806
RE: Crystal Cream & Butter Co. SITE CODE: 2098
404 W. Fremont CONTRACTOR #: 39000
Stockton, CA 95203
A review of the documents dated 1987 in our files indicate very little progress has been made to address
the written comments of the Central Valley Regional Water Quality Control Board (CVRWQCB) (see
copy attached). A soil gas survey was performed by Applied Geosystems in March 1988, however,
analytical results of the survey were not included in the report.
In light of the contamination documented in the groundwater from your former underground storage tank,
we are providing you the attached "Notice of Reimbursement" as official notification that your site has
been assigned to the San Joaquin County Public Health Services, Environmental Health Division's Local
UST Oversight Program for oversight of assessment and clean up activities at contaminated underground
tank sites.
Fees for review activities and inspections associated with contaminated site assessment activities will not be
billed by this office. The State Water Resource Control Board (SWRCB) will invoice you directly for
activities performed under the auspices of the Local UST Oversight Program. The goal of the site
evaluation performed by Public Health Services Environmental Health Division (PHS/EHD) is to certify
closures of underground tank leakage sites. Closure certification occurs only after mitigation activities are
completed.
A Letter or Acknowledgment is enclosed for your completion and return. Your acknowledgment of the
placement of your site into the Local UST Oversight Program as stated above is respectfully requested. In
addition, the individual or individuals authorizing the continued investigation shall provide identification
with respect to their title of authority.
/ b[
fJ A Division of San Joaquin County Health Care Service
