Box 388 • Stockton, CA 95201-0388 <br /> 209/468-3420 <br /> COP <br /> GUARDINO AND CRAWFORD <br /> 517 W FREMONT ST <br /> STOCKTON CA 95201 Mf `} t Z 9 ig <br /> RE: GUARDINO AND CRAWFORD SITE CODE: 1112 <br /> 517 W FREMONT ST <br /> STOCKTON CA 95203 <br /> The attached " Notice of Reimbursement' is official notification that your site has been <br /> assigned to the San Joaquin County Public Health Services, Environmental Health Division 's <br /> (PHS/EHD) Local UST Oversight Program for oversight of assessment and cleanup activities <br /> at contaminated underground tank sites. <br /> PHS/EHD sent you a letter dated March 7, 1994 advising you that we will be utilizing the <br /> State Water Resources Control Board (SWRCB) " Prioritization System for Leaking <br /> Underground Storage Tank Sites " ranking list in order to prioritize all the contaminated sites . <br /> The letter stated that based upon the priority ranking you will be directed to begin the <br /> rp required investigation . <br /> At this time PHS/EHD is directing you to begin the investigation to determine the impact <br /> to soil and groundwater from the unauthorized release of petroleum hydrocarbons from your <br /> UST as confirmed by laboratory analysis of soil samples obtained during the June 29, 1989 <br /> tank removal . <br /> Fees for review activities and inspections associated with contaminated site assessment <br /> activities will not be billed by this office. The State Water Resource Control Board (SWRCB) <br /> will invoice you directly for activities performed under the auspices of the Local UST <br /> Oversight Program. The goal of the site evaluation performed by PHS/EHD is to certify <br /> closures of underground tank leakage sites. Closure certification occurs only after <br /> mitigation activities are completed. <br /> In addition , the , " Regional Board Staff Recommendations For Initial Evaluation and <br /> Investigation of Underground Tanks" and "Appendix A " reporting format and the LUFT <br /> Manual shall be utilized as guidance documents in the preparation of work plans and/or <br /> reports. <br /> A Division of San Joaquin County Health Care Services <br />
