The work outlined in this plan to <br /> investigate contamination at the site has not occurred. The work as outlined in the <br /> approved Work Plan must be completed within 90 days of the date of this letter. If the <br /> work is not completed within that time frame the site will not be in compliance with <br /> PHS/EHD directives and reimbursement for corrective action costs from the State Water <br /> Resources Control Board Cleanup Fund may be jeopardized. <br /> A well permit application and $ 89.00 permit fee should be submitted to PHS/EHD as <br /> soon as possible. After the permit has been issued the work can be scheduled.
