22 May 2001 <br /> AGE-NC Project No 95-0167 <br />' Page 3 of 10 <br /> MW-6 are in Appendix A General lithology beneath the site is depicted in cross sectional view A-A ' <br /> on Figure 3 <br />' <br /> 24 MONITORING WELL COMPLETION <br />' Soil borings MW-5 and MW-6 were single-completed as ground water monitoring wells utilizing <br /> 2-inch diameter, schedule 40, blank polyvinylchloride (PVC) well casing and perforated, 2-inch <br />' diameter PVC well casing (0 020-inch slot) <br /> Each well was installed by placing a PVC well casing assembly through the hollow stem of the <br /> augers to the total depth of the boring The augers were incrementally raised as No 3 Lonestar Lapis <br /> Luster sand(filter pack) was slowly poured down the annulus between the well casing and the inner <br /> wall of the hollow stem auger The filter pack was placed adjacent to the screened casing from the <br /> bottom of the boring to a point approximately two feet above the top of the screen, following filter <br /> pack placement, a surge block was inserted into the well casing and was alternately raised and <br /> lowered (surged) repeatedly Approximately one foot of bentonite chips (annular seal) was placed <br /> above the filter pack and was hydrated with clean tap water The remaining annular space was <br /> backfilled with portland cement (grout seal) As-built monitoring well-construction diagrams are <br /> included in Appendix A <br /> 25 SURVEYING OF MONITORING WELLS <br /> On 07 November 2000, AGE surveyed a reference point on the casing tops of wells MW-5 and <br /> MW-6 to the nearest 0 01-foot relative to monitoring well MW-1, previously surveyed The results <br /> of the monitoring well survey are contained in Table I <br /> 2 6 WELL DEVELOPMENT ACTIVITIES <br />' On 15 November 2000, monitoring wells MW-5 and MW-6 were developed utilizing balling <br /> techniques and a two-stage submersible pump <br />' Prior to well development, total depth measurements were collected utilizing a Solinst water level <br /> indicator to measure the quantity of sediment settled at the bottom-at each'well The wells were- <br /> cleaned of fines with a steel-bailer Thereafter, ground water was purged from the bottom of each <br /> monitoring well utilizing a submersible pump until ground water was visually sediment free Purged <br /> ground water was containerized at the site in properly labeled DOT-approved model 17H 55-gallon <br /> drums Copies of monitoring well field logs for sampling/development activities are in Appendix B <br /> Advanced Gen,Environmentai Inc <br />
