It is my understanding that Mr. Jadwinder Singh is the new owner or operator of the underground storage tanks (USTs) at 6131 Pacific Avenue, Stockton, CA as of December 16, 2016. As a new owner, you must transfer all of your permits to the San Joaquin County Environmental Health Department (EHD) and enter the appropriate information into the California Environmental Reporting System (CERS). Each day that you operate without these being done, you are in violation. Please complete the following items within thirty days of the close of escrow [CA Health and Safety Code section 25284 (b) & (c)]: 1. Create an account and obtain facility access to the facility on CERS by contacting the lead user on the account and going to http://cers.calepa.ca.gov. Ensure that all required uploads are completed in a timely manner. 2. Underground Storage Tanks— Facility Form — Provide all current information. Log into CERS, update the required information, and submit for review by the San Joaquin County Environmental Health Department (EHD). (Note: Any change of information must be updated in CERS within thirty days of the changes.) (The form is appended.) 3. Underground Storage Tanks—Tank Forms— Provide all current information in CERS. Complete a separate tank form for each tank. (The form is appended.) 4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the appended forms, and upload in CERS. This document is required to be submitted annually. 5. Underground Storage Tank Monitoring and Response Plans: Upload in CERS. Retain an approved copy of these plans on site for future reference. (The forms are appended.) 6. If you are the owner, you must submit a notification form identifying the "Designated UST Operator" for each facility owned. 7. Report#5021: Update all information for all programs at this site including the UST and Hazardous Waste programs, sign and date. Bring with all other forms to our accounting office to pay for transfer fees. (A form is appended.) 8. Go to: http.//www.waterboards.ca.gov/cwohome/ust/regulatory/index.html#rqs to obtain the current statutes (Underground Storage of Hazardous Substances) and UST Regulations that you are required to have for your monitoring and response plan. 1
