A copy of that approval is attached. Your requested changes to <br /> the Overfill Protection was entered into CERS on 8-22-16. Please note that this <br /> document was approved by three prior inspectors. <br /> 2) Item#209 -The Designated Operator correct expiration date is 7/28/16 for the <br /> reports dated August 21, 2015 through August 2016_ A copy of that actual California <br /> UST System Operator Certificate is attached. The new expiration date is 7/27/18. <br /> In addition, the August 21, 2015, inspection report does show items Numbers <br /> 9,10,11 as 'NO" because the test results were not available on site. The fact the test <br /> results were missing is clearly indicated on both the front and back of the inspection <br /> form. The testing may have taken place but there was no documentation that the <br /> system passed therefore the "NO" was the correct indication. As you clearly stated - <br /> alI the required information must be-completed and document correctly. This is NOT <br /> a violation or repeat violation. <br /> 3) Item #211 – Designated Operator employee training not performed or log not kept_ <br /> All employee documentation was verified accurate and on site during the August 9t', <br /> 2016, inspection. The site currently staffs six employees which were all updated in <br /> July of 2016. The fact one employee did not have an actual start date does not <br /> negate current training. The Employee noted worked on and off at the site prior to <br /> 2004. A copy of the training log from 2009 to present is attached. <br /> 4) #309—Corrected on site. <br /> If you require any additional information or documentation from our site, please let me <br /> know.
