The designated operator failed to check and note the correct International Code Council operator expiration date on the August 2015 through July 2016 designated operator monthly inspection reports. The designated operator inadvertently listed the expiration date as 7/29/2016 instead of 7/27/2016. Also on the August 21, 2015 designated operator inspection report, the operator listed "no" for No. 9- Monitoring System Certification was completed within the past 12 months, "no" for No. 10 - Line Leak Detectors were tested/certified within the past 12 months, and "no" for No. 11- Spill bucket(containment structure) testing completed within the past 12 months. All three tests had been completed within the last 12 months on 8/8/2015. During the monthly inspection, the designated operator shall chec'K that all required information has been completed and documented correctly. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. These were corrected on site during the inspection. This is a repeat violation, Class II. 11 CCR 2715(f) Designated operator employee training not performed or log not kept. The designated operator employee training for Rouet Yoeum did not have the"Hire Date". The other employees had been hired in 1994, expect for Tami Nguyen who was hired on 12/1/2004. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a minor violation. -09 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. The Bravo box float and chain in the dispenser 1/2 under-dispenser containment for product-87 failed to stop the fiov, of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain and verified functionality. This was corrected on site. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by September 18, 2016.
