The designated operator <br /> inadvertently listed the expiration date as 7/29/2016 instead of 7/27/2016. Also on the August 21, 2015 designated <br /> operator inspection report, the operator listed "no" for No. 9- Monitoring System Certification was completed within <br /> the past 12 months, "no" for No. 10 - Line Leak Detectors were tested/certified within the past 12 months, and "no" <br /> for No. 11- Spill bucket(containment structure) testing completed within the past 12 months. All three tests had been <br /> completed within the last 12 months on 8/8/2015. During the monthly inspection, the designated operator shall chec'K <br /> that all required information has been completed and documented correctly. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. These were <br /> corrected on site during the inspection. <br /> This is a repeat violation, Class II. <br /> 11 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Rouet Yoeum did not have the"Hire Date". The other employees had <br /> been hired in 1994, expect for Tami Nguyen who was hired on 12/1/2004. The designated operator shall train facility <br /> employees for which he or she is responsible in the proper operation and maintenance of the UST system once <br /> every 12 months. The training shall include, but is not limited to <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a minor violation. <br /> -09 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo box float and chain in the dispenser 1/2 under-dispenser containment for product-87 failed to stop the fiov, <br /> of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the <br /> chain and verified functionality. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 18, 2016. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> Page 4 of 5 <br />
