The Certificate of Liability Insurance policy <br /> was dated November 21, 2013. The policy period was from December 1, 2013 through December 1, 2014. Current <br /> financial responsibility documents are required to be submitted annually. Immediately log into the California <br /> Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Daily alarms from the alarm history were not documented in the daily alarm log. The alarm not documented was a Q1: <br /> 87 Regular PLLD Shutdown on April 21, 2015. All alarms from the alarm history need to be documented in the daily <br /> alarm log and made available for review upon request of the EHD. Immediately began documenting all alarms from the <br /> alarm history in the daily alarm log and have available for review. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 87 STP sump sensor was raised approximately 2 inches off the lowest point of the sump and not located to <br /> detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the <br /> earliest possible opportunity. The sensor was relocated to the lowest point of the sump by the service technician. <br /> Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This was corrected on <br /> site. <br /> This is a Class II violation. <br /> 319 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87 STP sump on the east and west side . If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician <br /> removed 3 cups of liquid from the 87 STP sump. Ensure that all sumps and annular spaces are maintained free of <br /> liquid.
