Please feel free to contact me for any additional information. <br /> Code #4 —HSC25292.2—Attached are updated Certification of Financial <br /> Responsibility documents. <br /> Code #23c — CCR 2715 (c)(e) — On May 10, 2011, my site experienced an <br /> erroneous Gross Line Failure and subsequent PLLD Shutdown on the Premium <br /> tank. The alarm reset and self-cleared. Again on May 14, 2011, 1 experienced <br /> the same two alarms and Elite IV Contractors were called to investigate. Elite IV <br /> could not locate a problem, retested and cleared alarm with attached invoice <br /> #24937. On May 16, 2011, my Premium product would not pump. Elite IV <br /> returned with attached invoice #24939 and replaced a capacitor. The problem <br /> appeared to be resolved. On May 17, 2011, my DO inspected the site. The <br /> Alarm History report for the review period did not show the alarms in the tank <br /> history or the "L" sensor alarms. She did not print out the "O" alarms which <br /> clearly indicated the data above.
