Marigold Shell
6131 Pacific Avenue RECEIVED
Stockton, California 95207
August 22, 2011 AUG 2 3 2011
Mr. Garrett Backus, RENS ENVIRONMENTAL HEALTH
PERMITISERVICES
Senior Registered Environmental Health Specialist
San Joaquin County Environmental Health Department
600 East Main Street
Stockton, California 95202-3029
Reference: Inspection of Marigold Shell, 6131 Pacific Avenue, Stockton
Inspection Date: August 16, 2011
Dear Mr. Backus,
The following attachments and comments are in response to your inspection of
my facility. Please feel free to contact me for any additional information.
Code #4 —HSC25292.2—Attached are updated Certification of Financial
Responsibility documents.
Code #23c — CCR 2715 (c)(e) — On May 10, 2011, my site experienced an
erroneous Gross Line Failure and subsequent PLLD Shutdown on the Premium
tank. The alarm reset and self-cleared. Again on May 14, 2011, 1 experienced
the same two alarms and Elite IV Contractors were called to investigate. Elite IV
could not locate a problem, retested and cleared alarm with attached invoice
#24937. On May 16, 2011, my Premium product would not pump. Elite IV
returned with attached invoice #24939 and replaced a capacitor. The problem
appeared to be resolved. On May 17, 2011, my DO inspected the site. The
Alarm History report for the review period did not show the alarms in the tank
history or the "L" sensor alarms. She did not print out the "O" alarms which
clearly indicated the data above. Beginning with my August 2011 inspection, I
have required all alarms be printed and reviewed.
Please let me know if any additional documentation is required.
/Sincerely, )
Hung Thanh Tran
Owner
