The designated operator failed to visually inspect the under dispenser containers for leaks, water, or debris and did not mark"Yes"indicating that the spill buckets were inspected for leaks,water, or debris during the 1/27/17 monthly inspection. During the monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the reports. The designated operator failed to check and indicate that the alarm from the attached alarm history on the 6/14/17 designated operator monthly inspection report was responded to appropriately. The alarm was an 87 Overfill Alarm on 5/13/17. The designated operator's comments indicating how the alarms from the attached alarm history on the 1/27/17 and 9/23/16 designated operator monthly inspection reports were illegible. The alarms were an 87 Q1 PLLD Shutdown Alarm on 1/23/17, and an 87 Leak Alarm and 87 Q1 PLLD Shutdown Alarm on 8/31/16. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Submit a statement and supporting documentation that all of the above mentioned alarms were properly addressed. This is a repeat violation, Class Il. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 9/30/2017. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS. AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS FACILITY BACK INTO COMPLIANCE. THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
