During the monthly inspection, the designated operator shall visually inspect all spill containers and UDC <br /> sumps, and any tank top sump that had an alarm for which there is no record of a service visit. Ensure that <br /> designated operators performing monthly inspections at this facility are inspecting all of the required areas and <br /> noting it on the on the reports. <br /> The designated operator failed to check and indicate that the alarm from the attached alarm history on the 6/14/17 <br /> designated operator monthly inspection report was responded to appropriately. The alarm was an 87 Overfill Alarm <br /> on 5/13/17. The designated operator's comments indicating how the alarms from the attached alarm history on the <br /> 1/27/17 and 9/23/16 designated operator monthly inspection reports were illegible. The alarms were an 87 Q1 PLLD <br /> Shutdown Alarm on 1/23/17, and an 87 Leak Alarm and 87 Q1 PLLD Shutdown Alarm on 8/31/16. During the <br /> monthly inspection,the designated operator shall review the alarm history for the previous month, check that each <br /> alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. Submit a statement and <br /> supporting documentation that all of the above mentioned alarms were properly addressed. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 9/30/2017. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
