The monitoring plan on site is not current and not approved by the Environmental Health Department <br /> (EHD). The monitoring panel may be monitored by a remote service. Generate a statement to be <br /> attached to the monitoring plan. Indicate the name, address and telephone number of the remote <br /> service that is monitoring the alarms. Write out the procedure the remote service follows when the <br /> monitoring panel goes into alarm. The monitoring plan must be current and approved by the EHD. <br /> Complete and submit a copy of the monitoring plan for approval by the EHD by September 17, 2010. An <br /> approved copy will be returned for your records. <br /> 10 & 12. The 87 and 91 tank annular sensors failed to activate an audible and visual alarm when tested <br /> and, therefore, the tank was not being monitored. The EHD last witnessed correct operation of the <br /> audio / visual alarm system on August 6, 2009. The interstitial space shall be monitored with a <br /> monitoring system connected to an audible and visual alarm system. <br /> The testing technician failed to get audio / visual alarms when testing the 87 and 91 tank annular <br /> sensors. The technician then flipped a DIP switch on the ECPU2 circuit board to get the audible and <br /> visual alarm system to work. A service technician, working on the system on 11-11-09, also flipped a <br /> switch to show alarms on the monitoring panel. <br /> Maintain the monitoring panel to keep the audible and visual alarm system working correctly. Monitoring <br /> system sensors should trigger audible and visual alarms. Correct immediately. <br /> 23c. The designated operator failed to check and note the correct test dates for the secondary <br /> containment test on the July 2010 and August 2010 designated operator monthly inspection reports. <br /> The last complete set of secondary containment testing was done on September 10, 2007. The turbine <br /> sumps were retested on November 8, 2007. During the monthly inspection, the designated operator <br /> shall check that all required testing and maintenance for the UST system have been completed and <br /> document the dates they were done. Ensure that designated operators performing monthly inspections <br /> at this facility are including all of the required information on the reports. <br /> Note: Complete and submit a copy of the Return to Compliance Certification form with a statement of <br /> how each violation was corrected, including supporting documentation, by September 17, 2010. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector. l Recei�By: Title: <br /> 2 <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br /> i <br />
