Main St. <br /> Stockton, CA., 95202 <br /> As the responsible party, you were notified on November 6, 1998 that the illegally abandoned underground waste oil <br /> storage tank(UST)was to be brought into compliance by December 6, 1998 or a closure plan and all fees should be <br /> submitted to San Joaquin County, Public Health Services, Environmental Health Division (PHS-EHD), by December 6, <br /> 1998. Neither requirement has been complied with. <br /> On December 11, 1998 PHS-EHD sent you another notice and all of the forms to be completed and submitted in order <br /> to bring your site into compliance. The deadline was reset for December 21, 1998. As of the date of this notice,none of <br /> the requirements have been met and none of the required forms and fees have been received at this office. <br /> In order to discuss your failure to comply with the required UST Closure Plan submittal, a meeting has been <br /> scheduled for January 12, 1999 at 10am, 304 East Weber Avenue,Third Floor, Room 307, Stockton,California. <br /> Failure to appear or to respond prior to this meeting date may result in a referral to the San Joaquin County District <br /> Attorneys office for enforcement action. <br /> Sites that are not in compliance with directives issued by PHS-EHD jeopardize their ability to become and/or remain <br /> eligible to receive reimbursement for the State Water Resources Control Board's Clean Up Fund for underground tank <br /> investigation and remediation. <br /> If you have any questions or if you are unable to attend this meeting, contact Michael Infuma, Senior REHS,of my staff <br /> at(209)468-3454. <br /> Donna Heran, REHS, Director <br /> Environmental Health Division <br /> Michael J.
