Fremont Street <br /> Stockton CA 95203 <br /> In a letter dated June 13,2000,San Joaquin County Public Health Services Environmental Health <br /> Division.(PHS/EHD) directed you to continue sampling and submit monitoring reports on a <br /> quarterly basis in accordance with California Code Regulation,Title 23,Division 3,Chapter 16, <br /> Section 2652 (d). Thus far,PHS/EHD has received only one report,the Qua7oly Grn"rhutter <br /> Ma*oringard SanpILTRepon(the report) dated March 12,2001 prepared byJJW Geosciences,Inc. <br /> fflx� <br /> You are required to sample the monitoring wells and submit a quarterly monitoring report to <br /> PHS/EHD by November 25,2001. Failure to submit the report may result in a request for formal <br /> enforcement action from Central Valley Regional Water Quality Control Board(CVRWQCB) or San <br /> Joaquin CountyDistrict Attorney's office. <br /> In the above-mentioned June 2000 letter,PHS/EHD stated that if JJW believes the site is ready for <br /> closure,your consultant has the option of providing the information and justification for closure in <br /> the form of a closure report. PHS/EHD has not received this report to date. <br /> Please understand a closure report is not necessary in the determination that a site should be closed, <br /> but is requested by the CVRWQCB.. What is necessary is a 2000-foot radius well receptor survey. <br /> Submit the surveyincluding a map of the well locations,addresses of the well locations,and any <br /> known well construction details to PHS/EHD no later than November 25,2001. The surveycan <br /> be included in the quarterly sampling report and does not need to be a separate submission.
