California 95242 <br /> SUBJECT: WATER SERVICE FOR ASSESSOR'S PARCEL NO. 015-220-17 FROM <br /> MOKELUMNE ACRES MAINTENANCE DISTRICT <br /> Dear Mr, White: <br /> The Mokelumne Acres Maintenance District will serve the above referenced parcel with <br /> water provided all necessary improvements arc made to the water system, which <br /> comply with State and County standards. <br /> Annexation to the _Mokelumne Acres Maintenance District will be_rp uired and all <br /> applicable connection or buy-in fees, as shown on the attached Statement of <br /> Annexation Costs, shall be paid. Please provide a copy of the Grant Decd of the <br /> propErty to be annexed and checks or money orders for the amounts specified on the <br /> Statement of Annexation Costs. <br /> If any extension, connection, or placement of new facilities is required to connect <br /> to the existing supply, the cost shall be borne by the applicant. An improvement <br /> pian shall be submitted for review and approval_ A water meter (Scnsus brand or <br /> approved equal measurable in gallons) shall be installed and provided with attachment <br /> connector spud_ Any interruption to the existing water system shall be done by the Utility <br /> Maintenance Division only. if any interruption to the existing water system is necessary, <br /> please contact the Utility Maintenance Division at (209) 468-3090 to perform the task. In <br /> addition, the Utility Maintenance Division shall be contacted to inspect the connection to <br /> the, main water line prior to backfilling. <br /> If an existing well is to be destroyed, a permit from the Environmental Health Division <br /> ((209) 468-342.0) shall be obtained before water will be supplied. Destruction of the well <br /> shall be completed within 30 days of hook-up. If a well is to remain on the property, an <br /> approved reduced pressure backflow prevention device will be required. The applicant <br />
