Any material that meets or exceeds the reportable quantity shall be <br /> reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> The Hazardous Materials Inventory forms for the bleach, acetylene, propane, and oxygen were not complete and <br /> accurate. <br /> 900 gallons vs. 300 gallons of bleach was observed, the amount of acetylene (505 cubic feet) is reported in pounds <br /> vs. cubic feet, the amount of propane (96 gallons) is reported in pounds vs. gallons, and the amount of compressed <br /> oxygen (403 cubic feet) is reported in pounds vs. cubic feet. This information must be current and submitted as part <br /> of the business plan. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The maps did not show/address: <br /> 1. Loading areas. <br /> 2. Internal roads. <br /> 3. Adjacent streets. <br /> 4. Storm and sewer drains. <br /> 5. Access and exit points. <br /> 6. Emergency response equipment (fire extinguishers and absorbent). <br /> 7. Emergency shutoffs. <br /> 8. Evacuation staging area. <br /> 9. The map does not show the handling and storage areas for waste oil, sodium hydroxide, carbon dioxide (east <br /> wall of facility)and Freon; the map does not show the actual location of the propane (on the ground just north/west <br /> of the boiler room). <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as "NA".
