z
SMTH SITE SAFETY PLAN
FACILITY BACKGROUND
One 2000 gallon underground leaded gasoline storage tank was removed from the site in
December 1988. The site was excavated to a depth of approximately 14 feet below grade.
On December 5, 1988, one soil sample was collected from each end of the tank at a depth
of approximately 14 feet below grade. According the chain of custody, the samples were
received by FGL Environmental Laboratory in "sealed condition" on December 7, 1988.
Analytical reports were prepared by the laboratory on December 20, 1988. Laboratory
analyses of the samples detected toluene at 27 parts per billion (ppb) in both samples and
total lead at 8 and 10 parts per million (ppm), respectively.
WORK PLAN
Task One: SMTH will supervise the collection of four soil samples beneath the former
underground fuel storage tank. The samples will be collected by Kvilhaug
Well Drilling and Pump Co. Inc. (C57# 482390) using the direct push
technology. Discrete soil samples will be collected beneath each end of the
former tank location at depth of approximately 15 and'20 feet below grade.
Samples will be collected using the Geoprobe 5400 Soil Probe Unit, a dual-
rod sampling technique which will enable collection of undisturbed soil
samples at the desired depth. Subsequent to sample collection, any void
created by the Soil Probe Unit will be grouted to the surface with a neat
cement grout. Any soil cuttings generated during sampling activities will be
contained on site in Department of Transportation (DOT) approved drums
.pending analytical results of soil samples.
1'AGE 2
