SAN10 A Q U I N Environmental Health Department <br /> 0 U hJ 1 Y <br /> Complaint Record Daily Time and Activity Summary from 1/1/2014 to <br /> 2/17/2060 for C00051327 located at <br /> 1535 E PESCADERO AVE <br /> Activity Date Recorded by <br /> 12/10/2019 Initial called received at approximately 1:00 pm PE 2546 RUVALCABA <br /> 90 min SC 544 <br /> Contacted facility, Marc Murdock, DC Manager at approximately 3:00 pm. Mark <br /> stated that the cleanup had concluded and that the waste was in a drum. He <br /> stated that the original container of 50 gallons had been placed in an overpack <br /> drum and may be shipped to its final destination, pending instructions from the <br /> shipper. Marc was able to provide UN1418 from the shipping documents. This <br /> number was looked up in the 2016 emergency response guidebook it appeared <br /> facility had taken suggested actions by contacting professionals to perform the <br /> cleanup. A spill contingency plan for the facility was verified in CERS. It was stated <br /> that an environmental cleanup company was contacted and had performed the <br /> cleanup. The waste was containerized and deemed clean. <br /> Had conversation with lead senior regarding the incident. <br /> Contacted the reporting parry, Philip Stark with Hazmat One to try and get the <br /> cleanup company information. It was reported that Alcon Environmental had <br /> responded. The contact was give as Nolan. <br /> Nolan was called at(916) 701-3376 and pictures of the incident were requested. <br /> Pictures of the incident were received. <br /> Pictures were shared with lead senior. Decision was made to not go to the facility. <br /> A question of whether or not the material had leaked outside of the trailer was <br /> posed by lead senior from one of the pictures. Nolan was emailed and asked <br /> about this <br /> 12/11/2019 Elena Manzo (Lead Senior REHS) requested I go to YRC Freight location and PE 2546 GAITAN <br /> 90 min check the hazardous waste storage. SC 544 <br /> Talked to Cesar Ruvalcaba (RENS) about the complaint. <br /> Contacted Marc Murdock, DC Manager for YRC Freight. <br /> (209)405-1168 <br /> Let him know that I will be stopping by to inspect the hazardous waste storage <br /> area of the magnesium alloy powder. He stated that he will greet me at the gate <br /> when I arrive. <br /> Met with Marc, he took me to the magnesium alloy powder and waste debris <br /> storage area. Magnesium was observed to be contained with no signs of leaks or <br /> damage to container. The containers are located indoors. <br /> Stated that they are waiting for determination on whether or not the magnesius <br /> alloy powder is a waste or a product. <br /> Left to EHD. <br /> 12/11/2019 Generated photo log, printed out photo log and provided to Cesar Ruvalcaba PE 2546 GAITAN <br /> 24 min (REHS).
