The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http : //cers, calepa . ca . gov/, enter the correct or updated information , and submit to the EHD for approval . <br /> This is a Class II violation. <br /> 11 HSC 25505 (a) (4) Failed to provide or document initial and /or refresher training to appropriate personnel . <br /> Documentation showing initial and annual refresher training was not available at the time of inspection . The <br /> business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher <br /> training . All employees shall be trained in safety procedures in the event of a release or threatened release of a <br /> hazardous material , including , but not limited to , familiarity with the following plans and procedures : <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency . <br /> ( B ) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property, or the environment . <br /> (C) Evacuation plans and procedures , including immediate notice , for the business site . <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a Class II violation . <br /> 12 HSC 25505 (c) Failed to have the business plan readily available to facility or EHD personnel . <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures , the inventory of information required by this article , and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section . Immediately make arrangements to have the business <br /> plan readily available for review. <br /> This is a Class II violation . <br /> Overall Inspection Comments : <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation , and any supporting <br /> paperwork , within 30 days of receiving the completed inspection report . <br /> Mr. Nguyen has operated this facility since 2018 . <br /> Starting September 1 , 2018 , all in- office CERS help will be provided at EHD hourly rate ($ 152 ) .
