Fxcept as specifically provided otherwise by this license, the licensee shall possess and use radioactive material <br />described in Items 6, 7, 8 and 9 of this license in accordance with statements, representations, and procedures <br />contained in the documents listed below. The Departrnenes regulations shall govern unless the statements, <br />representations, and procedures in the licensee's application and correspondence are more restrictive than the <br />regulations. <br />(a) The application with attachments dated February 27, 1991, signed M. H Gartin. <br />(b) The letters with attachments dated January 20, 1994 and January 26, 1994 both signed by Bruce W. <br />Houghton regarding procedures. <br />(c) The letterwith attachment dated Match 25,1997, signed by Cmdy Dearborn, regarding company merger <br />and name change. <br />14. The Radiation Safety Officer in this program shall be ?Lith Galvan. <br />15. The licensee is authorized to dispose of BACTEC culture vials (containing Carbon-14) as nonradioactive <br />waste.
