78. Failure to clean and containerize broken lamp:
."Failed to clean and containerize the broken pieces of a fluorescent lamp observed in
the workshop" (Exhibit 10). An immediate corrective action was performed by
containerizing the broken lamp. Long-term practice: We will ensure proper handling
and storage of the lamps. We will conduct universal waste refresher training during Q1
once all the support staff is back on site.
reS-/v'r r,s „01
0 HIUARDOUS WASTE I
Exhibit l0a
.IM.. 1un, ilri
111/lllf
89. Empty HW/Hazardous Material containers were not marked with the date they
were:
•"15 empty 55 gallon drums near the used oil storage area outdoor" (Exhibit 11).
• "7 empty 55 gallon drums near the outdoor lathes" (Exhibit 11). All drums were
evaluated and sorted. Some drums were partially filled and therefore were not
removed because they contain materials that we will be consumed in the future.
They are all labeled with original label. Some drums had been purchased specifically
for use as Hazardous Waste storage containers. These drums were stored near the
used oil area. Although these drums looked weathered, they were new and had
never been used to contain any material, including hazardous waste. Thirteen (13)
of these empty drums have been removed from the facility by Van De Pol. (Exhibit
11 a). Two (2) of the empty drums remain on site. Enclosed please find Packing List
Document from Van De Pol to confirm removal of the empty drums from the site.
See link for Van De Pol doc.pdf.
