Page 2 <br /> Project No. 989.2 <br /> February 25,2005 <br /> In an attempt to further define the limits of soil and groundwater contamination three (3) <br /> GeoProbe borings were placed on the site on August 25, 1998. The results of the work are <br /> outlined in the Hunter report: Interim Report Covering Results of Site Investigation: <br /> Placement of Soil/Water Probes— September 28, 1998. <br /> On August 15, 2000, three (3) additional groundwater-monitoring wells were installed at the <br /> site. Details of the work are discussed in Hunter report: Interim Report Covering Results of <br /> Site Investigation; Placement of Soil/Water Probes & Groundwater Monitoring Results <br /> 16- Valley Wholesale Drug Co. —October, 2000. <br /> On October 5, 2001, VWDC received a directive from Jeffrey Wong, SJCEHD. The letter <br /> required VWDC to define the extent of the contamination plume and submit a work plan to <br /> SJCEHD by November 20, 2001. Geological Technics Inc. (GTI) requested an extension for <br /> the work plan submittal. On November 5, 2001, Mr. Wong granted the extension to <br /> December 5, 2001. <br /> The work pian was submitted on November 21, 2001 and was approved by Mr. Jeffrey Wong <br /> in a letter dated February 27, 2002. <br /> During the third quarter of 2002 GTI installed one deep and three shallow monitoring wells at <br /> the VWDC site. Soil samples were collected during the installation of the wells and the <br /> monitoring wells were developed 72 hours after installation. The new wells were surveyed in <br /> accordance with AB2886 and placed on the existing groundwater monitoring routine. <br /> On October 31, 2002, GTI submitted the Site Characterization — Monitoring Well <br /> Installation and 3rd Quarter Groundwater Monitoring Report discussing the installation of <br /> the new wells and characterize the site conditions. The report recommended two additional <br /> quarters of monitoring and then considers the site for closure. No response was given to the <br /> recommendations in this report. <br /> On January 21, 2003, GTI prepared and submitted a Work Plan for Geophysical Analysis. <br /> On February 17, 2003 GTI requested a response from SJCEHD. In a letter dated March 8, <br /> Lo 2003, Mr. Wong approved the work plan. <br /> On December 17, 2003, GTI prepared and submitted a Site Conceptual Model Report (SCM) <br /> again requesting that the site be considered for closure. On February 16, 2004 GTI requested <br /> a response from SJCEHD on the recommendations in the SCM. In a letter dated March 12, <br /> 2004 Mr. Wong denied the request for closure and directed VWDC to submit a work plan to <br /> L, define the extent of the groundwater contamination. <br /> On March 29, 2004, GTI prepared and submitted a Work Plan for Additional Site <br /> �.
