Fremont St <br /> Stockton, CA 95203 <br /> San Joaquin County Public Health Services, Environmental Health Division, (PHS-EHD) has evaluated the <br /> recent product line replacement report submitted by Brown and Caldwell and has the following comments. <br /> Petroleum Hydrocarbon impacted soils removed under a work plan approved by PHS-EHD on May 15, <br /> 1995 may have impacted the shallow groundwater beneath the site. In order to verify groundwater <br /> conditions in the west island area at the northern edge (sample T14 area), PHS-EHD will require a <br /> groundwater monitoring well to be installed. <br /> Only one well will be required at this time. A work plan that addresses the well construction and location <br /> should be submitted to PHS-EHD for review. In addition, a well installation permit application along with a <br /> $89 application;fee should be submitted with the work pian. <br /> Please submit your plan by June 15, 1996 and be prepared to start well installation procedures-by July 1; <br /> 1996. <br /> This well should be sampled on a quarterly basis for Total Petroleum Hydrocarbons as Gasoline and the <br /> BTEX constituents as well as MTBE.
