(Quik Stop), Horizon Environmental Inc. <br /> (Horizon) is forwarding the enclosed Well Permit Application Form and attached Licensed <br /> Contractors Declaration (LCD) Form for the three proposed groundwater monitoring wells <br /> at the above-referenced site in Stockton, California (Figure 1). On behalf of Quik Stop, <br /> Horizon previously submitted the Work Plan for a Preliminary Site Assessment (Horizon, <br /> October 27, 1999) to the San Joaquin County Public Health Services - Environmental Health <br /> Division (SJCPHS-EHD). On November 1, 1999, Quik Stop received approval of the <br /> proposed work from the SJCPHS-EHD, and wishes to proceed with the scope of work at the <br /> site. The location of pertinent site features and the proposed groundwater monitoring wells <br /> are shown on the enclosed Site Plan (Figure 2). <br /> We understand that the appropriate fees are $89.00 per application per site. A check for the <br /> Well Construction Application fees of $89.00 has been enclosed with this letter. The <br /> drilling subcontractor will be V&W Drilling of Rio Vista, California (C-57# 720904). The <br /> proposed work has been scheduled for Thursday, November 18 1999.
