An' email will suffice, or you may send the document by <br /> US Mail. <br /> E <br /> If you chose to keep the wells, ,a monitoring and reporting plan is required and <br /> billing information must be established so that the EHD can recover costs <br /> associated with the sampling inspections and report reviews. <br /> If well destruction is chosen, the EHD will need an email/mailed document that <br /> details the methods and materials you plan to use to complete the well <br /> destructions. I have discussed the San Joaquin County Well Standards <br /> (requirements) with Mr` Smith already concerning the minimums items/methods for <br /> clean' versus still 'contaminated' monitoring well destruction methods. Please <br /> provide the date you plan to do this when you have finalized it with the driller. A <br /> I' Well Permit Application Form and $89 fee is required for each parcel (2) where <br /> monitoring wells will be destroyed, as well as one for the City right of way for the <br /> well in the City property. Please ensure that the a copy;of the City encroachment <br /> permit is submitted with the.boring permit application and a copy of the private <br /> parcel (Church) access agreement is also submitted.. <br /> I Upon sucessfull destruction of all the wells and signed copies of the proper <br /> disposal of the waste produced by this process and ALL investigative waste still <br /> onsite, the EHD will issue the formal No Further Action required letter and show the <br /> site as closed on the State database. <br /> If you plan to keep the.wells and continue to monitor and sample and report the <br /> findings to the EHD, the official No Further Action letter can be issued after.the <br /> Monitoring Plan is approved by the EHD and the billing information is on file. <br /> i <br /> 2/17/2010 <br />
