Perry Pineda and Gurginder Singh: <br /> The San Joaquin County Environmental Health Department (EHD) has received and reviewed <br /> Well Destruction Work Plan (Work Plan), dated 01 August 2014, prepared and submitted by <br /> your consulting firm, Conestoga-Rovers & Associates (CRA), on your behalf for the above- <br /> referenced site. By letter dated 18 June 2014, the EHD had directed the submittal of a scope of <br /> work proposal that detailed the conditions and construction of the wells and the methods and <br /> materials to be used to destroy the wells. <br /> In the Work Plan, CRA proposes to destroy monitoring wells S-1, S-1-85, S-3-84, S-4, S-4-83 <br /> and S-6 by drilling out the wells to their original depths using the same size augers that <br /> originally were used to install the wells. CRA proposes to destroy monitoring wells S-2, S-2-85, <br /> S-3 and S-5 by pressure grouting as follows: <br /> • Remove the well boxes; <br /> • Remove the surrounding soil of the top three feet of each well location using an air-knife <br /> to form a borehole that is at least one foot larger in diameter than the original borehole; <br /> • Remove the well casing at approximately two-and-a-half feet below surface grade (bsg); <br /> • Add cement grout to the well casing from total depth to the top of the casing using a <br /> tremie pipe until no additional grout can be added to the casing; <br /> • Apply pressure to the grout inside of the casing; then <br /> • Add grout so that the grout spills out of the top of the casing and forms a one-foot-thick <br /> layer of grout in the excavation. <br /> The EHD approves the Work Plan with the following conditions: <br />
