Fremont Shell Station Page 3 of 3 <br /> 2494 East Fremont Street June 18, 2014 <br /> Stockton, California 95205 <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and Boring <br /> Permit Application form along with a Master File Record form, Licensed Contractor's Declaration <br /> form and appropriate fees for each parcel. The permit fee for the destruction of each well is $125, <br /> along with a $375 Inspection Activity fee per parcel. A review of the site records indicate that ten <br /> monitoring wells are installed onsite. A fee of $1,625.00 (10 x $125 + $375) will be required for <br /> the well destruction permits. <br /> The EHD will issue a closure letter once the sixty-day public participation notification requirements <br /> and opportunity for public participation in the case closure decision process have been completed <br /> in accordance with the LTCP, all wells have been properly destroyed and waste manifests have <br /> been submitted to this agency. <br /> If you have any questions, please contact Vicki McCartney at (209) 468-9852 or by email at <br /> vmccartney@sjcehd.com. <br /> Sincerely, <br /> Victoria L.
