Fremont Shell Station Page 3 of 3 <br /> 2494 East Fremont Street June 18, 2014 <br /> Stockton, California 95205 <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and Boring <br /> Permit Application form along with a Master File Record form, Licensed Contractor's Declaration <br /> form and appropriate fees for each parcel. The permit fee for the destruction of each well is $125, <br /> along with a $375 Inspection Activity fee per parcel. A review of the site records indicate that ten <br /> monitoring wells are installed onsite.
