An"Underground Storage Tank Certification of Installation/Modification" form shall be <br /> submitted by the owner or the owners'agent [pursuant to(CCR)2635(d)] before this <br /> upgrade will be considered complete by EHD. <br /> 7. Permit expires 180 Days from the approval date.
