Propane—the maximum daily amount is wrongly identified at 4.7 gallons vs. 96 gallons,the size of the <br /> largest container(8 gallons),the average daily amount, and the type of storage container(cylinders) <br /> elements have not been completed, and the storage pressure has wrongly been identified as"ambient" <br /> vs. "above ambient". <br /> • Violation #6,The site map has not been submitted to CERS and the email copy of the site map does not show or <br /> address the following: <br /> 1. North orientation. <br /> 2. Loading areas. <br /> 3. Internal roads. <br /> 4. Adjacent streets. <br /> 5. Storm and sewer drains. <br /> 6. Access and exit points from the street. <br /> 7. Propane handling and storage areas. <br /> 8. Emergency response equipment (fire extinguishers). <br /> 9. Emergency shutoffs. <br /> 10. Evacuation staging areas. <br /> 11. A legend to identify codes/symbols used on the map has not been provided (FE). <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #7, Emergency Response/Contingency Plan is not complete. <br /> - The local Unified Program Agency(UPA) phone number(209) 468-3420 has not been identified. <br /> - The location of the Emergency Response equipment and telephones has not been identified. <br /> • Violation #11—The training record does not show the required training elements (see 1, 2, 3, and 4 below) <br /> have been addressed. <br /> 1. Familiarity with the emergency response plans and procedures in the event of a release or threatened <br /> release of a hazardous material; (Emergency Response/Contingency Plan familiarization). <br /> 1 <br />
