R
ENVIRONMENTAL TECHNOLOGIES CORPORA710N
MT
August 5, 1997
P90865
Ms. Lori Shields
Diesel Performance, Inc.
2804 East Fremont Street
Stockton, California 95208-0387
Subject: Change Order No. 1 for Additional Work Performed for the Site Investigation at
2804 East Fremont Street, Stockton, California
Dear Ms. Shields:
On July 23, 1997, Smith Technology Corporation performed a site investigation at the above
referenced site. Ms. Mary Meays of San Joaquin County Public Health Services Environmental
Health Division was present during field activities and requested that additional work be
performed. The additional scope of work which was not provided for in Smith Technology's
original proposal No. P90865, dated May 29, 1997, included advancing additional borings and
submitting additional soil samples for laboratory analysis, and subsequently including the data in
the summary report. The scope of work is described in detail in the following paragraphs.
ADDITIONAL SCOPE OF WORK
Task 1: Soil Boring Installation
Smith Technology proposed to advance three soil borings to depths of 30 feet each employing
direct push technology and collect two soil samples from each boring (six total) for laboratory
analysis. This scope of work was included in Smith Technology's Workplan for Subsurface
Investigation which was approved by PHS/EHD on July 21, 1997.
During actual drilling, field conditions warranted that two additional borings, five total, be
advanced to provide adequate delineation of the site. The three original borings were advanced to
30 feet below ground surface (bgs) and the two additional borings were advanced to depths of 5
feet and 45 feet bgs. Ms. Meays was in agreement that the two additional borings were needed.
Task 2: Soil Sample Analysis
Following field activities, a list of the soil samples collected was provided to PHS/EHD per the
July 21, 1997 correspondence.
