I closed out four(127, 301, 403,406) of the open violations. Thirteen violations
remain open at this time. Two of the violations are from the tiered permit inspection. I have not received a response to
that inspection yet. I've attached a copy of the report. To close out those violations I still need:
-Return to Compliance certification filled out
-A copy of the written operating instructions for the wastewater treatment unit
-A copy of December 2018 records of the amounts and types of waste treated by the wastewater treatment unit
-A copy of December 2018 records that show the date, type, and quantities of the materials collected and laundered at
the facility
-A copy of records that demonstrate that the wash water conveyances and containers are constructed of materials to
ensure that they are impervious under the conditions of use
-A copy of records and procedures that show that the wash water conveyances and containers are visually inspected at
least twice a year to ensure that waste wash water is not leaking into the underlying soil
-A copy of training program records that demonstrate that the facility personnel are able to safely and properly handle
and clean the reusable soiled textile materials. The records should include the training content and record of employee
training
For the large quantity generator inspection report the statements indicate that everything will be done by
2/28/2019. So please provide an update by 3/4/2019. Here is what I need to close out those remaining violations:
