As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Owner/Operator Identification form has not been submitted annually to the California Environmental Reporting System (CERS).The last submittal of the The Business Owner/Operator Identification form was on 11/08/2017 . This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a minor violation. 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. Four 55 gallons drums(approximately 1200 pounds)of Floor Guard were found on site that have not been reported. The chemical inventory section has not been submitted annually to the California Environmental Reporting System (CERS). The last submittal of the chemical inventory was on 11/08/2017 Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. This is a repeat violation, Class II. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. A site map was not completed and submitted with all the required elements. The site map has not been updated with the current locations of the chemicals.The site map did not address the location of Loading Areas, internal roads, Emergency Response Equipment, and Emergency Shutoffs. The site map includes numbers and abbreviations that are not defined within the site map. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA".
