The regulations say that the operation plan shall include: <br /> (2) a copy of a map showing the general THHWCF perimeters and traffic pattern.The map shall include all of the <br /> information as specified in Sections 66270.14(b)(18)(A), (F), (G), (H), (J) and (L); (Referenced in section 17 but not <br /> attached to the Ops Plan) <br /> (11)training documents as specified in Section 66264.16(d); (These items were referenced as attachments in section 10 <br /> but not included: (1) name of the employee filling each job; (3) a written description, including a syllabus and/or outline, <br /> of the type and amount of both introductory and continuing training that will be given to each person filling a position <br /> listed under paragraph (d)(1) of this section; (4) employee-signed or-certified records that document that the training <br /> required under subsections (a), (b), and (c) of this section has been given to, and completed by, each employee. <br /> (15) copies of all local permits obtained for the operation of the facility; (Referenced in section 15 but not attached) <br /> (16) a copy of the written agreement between the property owner and the operator or the THHWCF; (Referenced in <br /> section 16 but not attached) <br /> (17) if the facility is to be managed by a contractor, a copy of the written agreement between the contractor and the <br /> operator of the THHWCF (Referenced in section 18 but not attached) <br />
