The regulations say that the operation plan shall include: (2) a copy of a map showing the general THHWCF perimeters and traffic pattern.The map shall include all of the information as specified in Sections 66270.14(b)(18)(A), (F), (G), (H), (J) and (L); (Referenced in section 17 but not attached to the Ops Plan) (11)training documents as specified in Section 66264.16(d); (These items were referenced as attachments in section 10 but not included: (1) name of the employee filling each job; (3) a written description, including a syllabus and/or outline, of the type and amount of both introductory and continuing training that will be given to each person filling a position listed under paragraph (d)(1) of this section; (4) employee-signed or-certified records that document that the training required under subsections (a), (b), and (c) of this section has been given to, and completed by, each employee. (15) copies of all local permits obtained for the operation of the facility; (Referenced in section 15 but not attached) (16) a copy of the written agreement between the property owner and the operator or the THHWCF; (Referenced in section 16 but not attached) (17) if the facility is to be managed by a contractor, a copy of the written agreement between the contractor and the operator of the THHWCF (Referenced in section 18 but not attached)
