MONITORING AND REPORTING PROGRAM ORDER NO, R5-2008-0149-010 BARNES TRUCKING FACILITY SAN JOAQUIN COUNTY (c) Any person discharging hazardous waste, as defined in Section 25117 of the Health and Safety Code, who knowingly fails or refuses to furnish technical or monitoring program reports as required by subdivision (b) of Section 13267, or who knowingly falsifies any information provided in those technical or monitoring program reports, is guilty of a misdemeanor, may be civilly liable in accordance with subdivision (d). (d)(1) Civil liability may be administratively imposed by a regional board in accordance with Article 2.5 (commencing with Section 13323) of Chapter 5 for a violation of subdivision (c) in an amount which shall not exceed five thousand dollars ($5,000) for each day in which the violation occurs. It is Hereby Ordered that the Discharger shall comply with the following Monitoring and Reporting Program requirements: GENERAL REQUIREMENTS 1 . All samples should be representative of the volume and nature of the discharge or matrix of material sampled. The time, date, and location of each grab sample shall be recorded on the legal chain-of-custody form. GROUNDWATER MONITORING 2 . Site wells are screened as high as 5 feet bgs and as deep as 30 feet bgs, with varying screen lengths and intervals. Each well is segregated below by: 1) the zone the well is screened within, 2) the category of well depth (deep or shallow), and 3) the well treatment category (treatment zone, upgradienbbackground, transition zone, and compliance zone). Below are two tables: one table showing this infonnation for each existing Site well and one table showing the same infonnation for the proposed wells.
