All <br /> work will be completed in accordance with Helmick and Lerner's Work Plan for Installation of <br /> Monitoring Well, dated February 6, 1998. This workplan was approved by San Joaquin County <br /> Public Health Services Environmental Health Division (PHS/EHD) in correspondence dated <br /> March 25, 1998. <br /> All work described in this workplan will be performed under the direction of a California <br /> Registered Professional Engineer. ATC's Site Safety Plan is contained in Attachment 1 and our <br /> Quality Assurance/Quality Control Plan is contained in Attachment 2. The Groundwater Well <br /> Installation Permit application is attached and the corresponding check(no. 191) is also attached. <br /> Once PHS/EHD has approved the Groundwater Well Installation Permit, ATC will schedule the <br /> installation of the groundwater monitoring well.
