Weber Street, Third Floor <br /> Stockton, California 95202 <br /> Subject: Site Safety Plan and Permits for Well Installation at Franklin High School, 300 <br /> North Gertrude Avenue, Stockton, California <br /> Dear Mr. Wong: <br /> ATC Associates Inc. is pleased to submit this Site Safety Plan and Well Installation Permit <br /> application on behalf of the Stockton Unified School District for the above referenced site. All <br /> work will be completed in accordance with Helmick and Lerner's Work Plan for Installation of <br /> Monitoring Well, dated February 6, 1998. This workplan was approved by San Joaquin County <br /> Public Health Services Environmental Health Division (PHS/EHD) in correspondence dated <br /> March 25, 1998. <br /> All work described in this workplan will be performed under the direction of a California <br /> Registered Professional Engineer. ATC's Site Safety Plan is contained in Attachment 1 and our <br /> Quality Assurance/Quality Control Plan is contained in Attachment 2. The Groundwater Well <br /> Installation Permit application is attached and the corresponding check(no. 191) is also attached. <br /> Once PHS/EHD has approved the Groundwater Well Installation Permit, ATC will schedule the <br /> installation of the groundwater monitoring well.
