Larry's Auto Page 2 <br /> 308 North Grant Street November 28, 2012 <br /> Stockton, California 95202 <br /> Submit a well installation permit application, encroachment permit issued by the City of <br /> Stockton, master file record (MFR) form, and fees to install the four previously approved SVE <br /> wells, two previously approved "shallow" IAS wells and two "deep' IAS wells in the north <br /> sidewalk of Miner Avenue to the EHD by 10 January 2013. Complete the well installations by <br /> 31 January 2013, and perform the pilot tests by 01 March 2013. <br /> By letter dated 05 August 2011, the EHD approved the following sampling schedule for this site: <br /> • Annually collect groundwater samples from monitoring wells MW-2, MW-4, MW-5, <br /> MW-6, MW-8, MW-9, extraction well EW-1, and vapor extraction well VW-5B; <br /> • Remove monitoring wells MW-1, MW-3 and MW-7 from the sampling schedule, <br /> however, continue to collect depth-to-water measurements from these three wells during <br /> the annual sampling event; 6 <br /> • Analyze the samples for total petroleum hydrocarbons as gasoline (TFtP-g); benzene, <br /> toluene, ethylbenzene, and total xylenes (BTEX); methyl tertiary-butyl ether (MTBE), <br /> tertiary-amyl methyl ether (TAME), and 1,2-dichloroethane (1,2-DCA); and <br /> • Include the analysis of total petroleum hydrocarbons as diesel (TPH-d) for groundwater <br /> samples collected from MW-4, MW-5 and MW-8. <br /> It does not appear that this sampling schedule is being followed based on the most recent report <br /> titled Semi-Annual Report— Third Quarter 2012, dated 09 November 2012.
