The following is an itemized list of underground storage tank violations that have not been addressed for KWIK SERV LODI BW 113* as of April 30 , 2020 . Open violations from January 09 , 2020 inspection Violation #206 - Failure to comply with UST operating permit conditions . OBSERVATION According to the replacement notifications sent to EHD on 9/ 10/2019 and 11 / 1 /2019 , the 87 product leak detector was replaced on 9/9/2019 and the 91 product leak detector was replaced on 10/27/2019 ; work orders or testing documentation indicating the replacement were not found on site at the time of the inspection . REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed , trained , and certified contractor pursuant to the EHD ' s policy and a permit obtained within one business day from the EHD . All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. CORRECTIVE ACTION : Immediately obtain a permit from the EHD for the replacement of the leak detectors on the 87 and 91 tanks . A " Failure to Obtain a Permit" penalty applies . ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #209 - Failure to conduct the designated UST operator visual inspection at least once every 30 days . 1 ) OBSERVATION: The designated operator failed to check and note the correct test dates for the Secondary Containment Testing on the 1 /2019 - 1 -2020 designated operator inspection reports . Secondary Containment Testing was conducted on 1 / 16/2018 . REGULATION GUIDANCE: During the inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done . CORRECTIVE ACTION : Ensure that designated operators performing inspections at this facility are including all of the required information on the reports . 2 ) OBSERVATION Designated operator reports were not signed within the required time frame . -The 1 / 11 /2019 designated operator report was not signed by the owner/operator . REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection , the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 48 hours of being provided a signed copy of the report , the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report , acknowledging the identified compliance issues . CORRECTIVE ACTION : Ensure all designated operator inspection reports are completed and signed within the required time frame in the future . Page 2 of 3
