The following is an itemized list of underground storage tank violations that have not <br /> been addressed for KWIK SERV LODI BW 113* as of April 30 , 2020 . <br /> Open violations from January 09 , 2020 inspection <br /> Violation #206 - Failure to comply with UST operating permit conditions . <br /> OBSERVATION According to the replacement notifications sent to EHD on 9/ 10/2019 and 11 / 1 /2019 , the 87 <br /> product leak detector was replaced on 9/9/2019 and the 91 product leak detector was replaced on 10/27/2019 ; work <br /> orders or testing documentation indicating the replacement were not found on site at the time of the inspection . <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed , trained , and certified contractor pursuant to the EHD ' s policy and a permit <br /> obtained within one business day from the EHD . All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION : Immediately obtain a permit from the EHD for the replacement of the leak detectors on the <br /> 87 and 91 tanks . A " Failure to Obtain a Permit" penalty applies . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #209 - Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days . <br /> 1 ) OBSERVATION: The designated operator failed to check and note the correct test dates for the Secondary <br /> Containment Testing on the 1 /2019 - 1 -2020 designated operator inspection reports . Secondary Containment <br /> Testing was conducted on 1 / 16/2018 . <br /> REGULATION GUIDANCE: During the inspection , the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done . <br /> CORRECTIVE ACTION : Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports . <br /> 2 ) OBSERVATION Designated operator reports were not signed within the required time frame . <br /> -The 1 / 11 /2019 designated operator report was not signed by the owner/operator . <br /> REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection , <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 48 <br /> hours of being provided a signed copy of the report , the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report , acknowledging the identified compliance <br /> issues . <br /> CORRECTIVE ACTION : Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future . <br /> Page 2 of 3 <br />
