readings in the field suggested that the system was not removing hydrocarbons as predicted. Potential <br /> problems such as short circuiting and ineffective vacuum or flow rate were discussed. <br /> On January 20, 1998 PHS/EHD staff became aware of significant errors regarding original calculations of <br /> contaminant mass residing in the soils at the above referenced site. PHSIEHD staff reviewed and re- <br /> evaluated the"Site Characterization"report by Geological Technics Inc. dated October 11, 1996. Table 3, <br /> Mass Balance Calculations, from the October 11, 1996 report was re-evaluated by PHS/EHD site <br /> mitigation staff. The calculation methods employed in this table could not be readily determined realizing <br /> that this appears to be an electronic spreadsheet. PHS/EHD staff contacted Geological Technics Inc. by <br /> telephone to discuss these findings on January 20, 1998. During this telephone conversation <br /> representatives from Geological Technics Inc. agreed that mass balance calculations found in the October <br /> 11, 1996 report were likely in error. <br /> PHS/EHD site mitigation staff have determined that the report included a review and signature by a <br /> registered professional, however, the mass balance calculations presented are likely inaccurate and grossly <br /> overestimate the actual contaminant mass residing beneath the former underground storage tank at the <br /> above referenced site. During the January 20, 1998 telephone conversation with Geological Technics Inc. <br /> staff, PHSIEHD verbally requested a written description of events at the site including a corrected and <br /> detailed estimate of contaminant mass at this site. <br /> PHS/EHD has not received any documentation from your consultant, Geological Technics Inc., to date as <br /> verbally requested on January 9, 1998 and again on January 20, 1998 regarding the performance of the <br /> SVES and errors or omissions regarding representation of data,and therefore health risk in earlier reports. <br /> PHS/EHD formally directs the responsible party at this site to provide this agency with a comprehensive <br /> chronology of events for the operation of the SVES and amendments to all reports as needed to more <br /> accurately characterize the contaminant distribution at the above referenced site. Only after reviewing <br /> corrected reports can this agency provide comments regarding future requirements at this site. PHS/EHD <br /> also directs the responsible party at this site to discontinue the operation of the SVES until all operational <br /> and reporting issues are resolved. PHS/EHD also recommends that the SVES remain on-site until all site <br /> information can be reconciled. <br /> PHS/EHD will require the amendments requested above to clearly identify and estimate the mass of each <br /> contaminant of concern (Benzene, Toluene, Xylene, Ethylbenzene, Methyl-tertiary butyl ether, and Total <br /> petroleum hydrocarbons as Gasoline)which reside in the subsurface environment at this site. These mass <br /> balance estimate amendments shall show all calculation methods. All previous and future reports <br /> Generated by this consulting Turn (GTI) at this site including recommendations and conclusions shall also <br /> be amended as needed.
