I looked into this last week but had to <br /> go out of state for a funeral returning last night. I had Marvin gather the information required. Marvin indicated at the <br /> time in June, 2017,you indicated he just needed to input the information into CERS. He didn't realize he also needed to <br /> provide the certification and photos showing the corrections. <br /> At the time of the inspection there were barrels and bags in our facility which were removed immediately. We have not <br /> had any of those products in quantities required to report in our facility since they were removed in June 2017. <br /> Our facility was entered into CERS at that time. Marvin indicated when he answers no to the questions because they <br /> don't apply to us the facility information goes away. I tried it,too, and the same thing happened to me last week. If we <br /> are doing something incorrectly please let me know so we can fix that issue. <br /> I have attached the following documents: <br /> 1. Statement of Compliance Certification on Hazardous Materials Program <br /> 2. Consolidated Emergency Response/Contingency Plan <br /> 3. Hazardous Materials Business Plan Training Program. (We do train our employees even though we do not have <br /> the quantity required for the CERS program) <br /> 4. Map and site photos <br /> 5. Photos of the warehouse showing the cleaned up areas attached to the violation <br /> 6. Screen shots of the CERS information <br /> 7. Two return to compliance certifications...one dated July 2017 when the work was actually completed and one <br /> dated last week once he gather the attached documents. <br /> As you indicated it slipped through the cracks in your office, I also want to apologize for our oversight in providing the <br /> completed documentation required by the inspection. Marvin believed completing the information online was <br /> sufficient.
