If a workers compensation loss occurs, the Corporate Workers Compensation Analyst <br /> (Donna Miller, 661-904-0978)handles all communication with the carrier. <br /> This project does not participate in an OCIP or project-specific insurance program. Box 25977 <br /> Shawnee Mission, KS 66225 <br /> (619) 688-3771 <br /> California Specific Policy Number: WC1357489(American Home Assurance) <br /> Policy Effective Dates: 1/1/2008 through 1/1/2009 <br /> 4.11.6 Medical Surveillance Programs and/or hbnitoring <br /> All personnel engaged in activities that results in the exposure to chemicals at or above the <br /> OSHA Permissible Exposure Limit (PEL) or wear a respirator for more than 30 days in a year, <br /> must comply with 29 CFR 1910.120(f) — Medical Surveillance. All personnel who wear a <br /> respirator must be medically qualified by a physician, trained and fit-tested on an annual basis, <br /> even if they are not required to participate in a medical surveillance program under 29 CFR <br /> 1910.120(0. <br /> Based on the activities listed in Section 2.1, the following potential hazards or activities are <br /> associated with this project. As a result, medical surveillance will be required as listed below: <br /> Name/Job Classification Hazard/Activity Medical Surveillance/Training <br /> Onsite Personnel Noise -Working around drilling equipment If noise exposures exceed 85 decibels over an 8- <br /> being used for the installation of monitoring hour time weighted average,an employee must <br /> wells and soil borings participate in a Hearing Conservation Program. <br /> Onsite Personnel Chemical exposures — TPH compounds, If an employee is exposed at or above the <br /> and VOCs will be monitored using air Permissible Exposure Limit (PEL) of a chemical for <br /> monitoring equipment, both direct read out more than 30 days in a year,they must participate in <br /> and lab analyzed equipment will be used a Medical Surveillance Program. <br /> through out the site. <br /> Onsite Personnel Respirator use — This is not anticipated.
