<br /> 6.1 SITE RISK ANALYSIS
<br /> Before work begins, Project Managers lead a team that performs a risk analysis at each work site
<br /> to identify hazards that require specific control measures. During weekly progress meetings, the
<br /> Project Engineer and subcontractors submit written summaries of upcoming work tasks and
<br /> associated risks and control measures to the Project Manager using the Activity Hazard Analysis
<br /> (AHA) form provided as Exhibit 6-1. The weekly summaries identify upcoming mobilization or
<br /> demobilizations tasks, audits and inspections, competent person changes, training and new
<br /> activities requiring an AHA. Subcontractors add activities to these summaries at least two weeks
<br /> in advance of the work. Potential hazards are listed below.

<br /> Toluene 8.64 mg/kg fatigue,weakness,confusion,dizziness,headache,dilated pupils,
<br /> excessive tearing,nervousness,muscle fatigue,paresthesia,
<br /> dermatitis,liver and kidney damage
<br /> Ethylbenzene 3.77 mg/kg Colorless,liquid with heavy distinctive aromatic odor. Causes Eye,
<br /> skin,and mucous membrane irritation;headache;dermatitis;narcotic;coma
<br /> Colorless liquid with aromatic odor. Irritates eyes,skin,nose,and
<br /> throat.Causes dizziness,drowsiness,staggered gait,lack of
<br /> Xylenes 13.9 mg/kg coordination,irritability,excitement,corneal irregularities,
<br /> conjunctivitis,dermatitis,anorexia,nausea,vomiting,abdominal
<br /> pain,and olfactory and pulmonary changes.Also targets blood,liver,
<br /> and kidneys. Mutagen and experimental teratogen.
<br /> Eye,skin and mucous membrane irritation; dermatitis,headache,
<br /> TPH 120 mg/kg fatigue,blurred vision,dizziness,slurred speech,confusion,
<br /> convulsions,chemical pneumonia on aspiration,possible liver and
<br /> kidney damage
<br /> Total PAH Pale-yellow crystalline or powder;Can cause skin irritation with
<br /> Benzo(a)pyrene (individual results rash and/or burning sensations,irritation and burning of the eyes on
