The following is an itemized list of hazardous materials violations that have not been <br /> addressed for KENTUCKY FRIED CHICKEN as of March 19, 2020. <br /> Open violations from September 25, 2019 inspection <br /> Violation #6 - Failed to complete and/or submit a site map with all required content„ <br /> The site map was not complete as part of the business plan. the site map was hard to read. The site map page is to <br /> dark and can not be easily read. <br /> The site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br /> drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br /> storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log <br /> into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or <br /> updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 -Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures are not complete. The evacuation staging area is not listed on line E3 Please <br /> add the location of the evacuation staging area to line E3. <br /> .
